Rental Equipment Loss Damage Policy

As an additional service to our customers, Gammie HomeCare offers an optional Loss Damage Waiver (LDW).  The LDW will help reduce the cost to you for loss or damage to the equipment while on rent under normal working conditions.  The charge of $14.99 will be automatically applied at the time of rental unless you “Decline” where indicated below on this form.

This Loss Damage Waiver covers:

• Theft: 90% of the retail purchase price of new equipment (customer is responsible for 10%)
• Damage: 90% of the cost of repairs to the equipment (customer is responsible for 10%)

• Abuse, neglect, misuse, intentional damage and/or carelessness
• Use of machine beyond its rated capacity.
• Mysterious disappearance or theft not documented with applicable public authorities.
• Water or bodily fluid damage
• Damage caused by overturn, tipping or dropping.
• Fraudulent or dishonest acts by any authorized representative of the lessee or by others to whom the property is entrusted.
• Your failure to properly secure the equipment such as leaving keys readily available to any unauthorized operator.
