1. Introduction and Scope

AdaptHealth LLC and its affiliates and subsidiaries (collectively, "we," "us," or "our") are committed to protecting the privacy and security of your personal information. This Privacy Notice describes how we collect, use, disclose, and protect information about you when you visit any website, our MyApp+ mobile application, or digital property owned or operated by us that links to or references this Privacy Notice (collectively, the "Services"), or otherwise interact with us. It also describes your rights regarding your personal information and how to exercise them. Where a particular affiliate, subsidiary, or digital property operates under a separate or supplemental privacy notice, that notice will govern to the extent it conflicts with this Privacy Notice.

HIPAA NOTICE: If you are accessing our Services in connection with the provision of health care, some of your health information may constitute Protected Health Information (PHI) subject to the Health Insurance Portability and Accountability Act (HIPAA). Our HIPAA Privacy Notice, which governs our use and disclosure of PHI, is available at https://adapthealth.com/pages/hipaa-privacy-policy and is incorporated into this Privacy Notice by reference. To the extent of any conflict between this Privacy Notice and our HIPAA Privacy Notice with respect to PHI, the HIPAA Privacy Notice governs.

2. Information We Collect

We may collect personal information in the following categories, depending on how you interact with our Services:

• Basic Identifiers: Name, email address, date of birth, username, password, phone number, billing address, shipping address
• Health and Medical Information: Prescription information, insurance information, appointment scheduling data
• Commercial/Transactional Information: Order history, purchase records, returns, service interactions
• Website Device, Usage Data, Diagnostics: IP address, browsing history on our sites, search queries, click data, session duration, device and browser type, crash logs, performance data
• Mobile App Device, Usage Data, Diagnostics: Device ID, OS version, app version, push notification token, in-app events, session duration, crash logs
• Geolocation Data: Approximate location (from IP address)
• Communications: Messages, emails, or other communications you send us, support tickets
• Job Applicant Data: Full name, email, phone number, resume/CV, work history, education credentials

We do not directly collect payment card information; our payment processor collects it directly from you and processes it under its own privacy policy.

We may collect the following additional categories of information through our MyApp+ mobile application:

• Camera: If you grant access, the App may use your camera to photograph an insurance card, scan a registration QR code, or a driver's license barcode. We do not record video without your knowledge. Camera access is used only while the App is in the foreground and you have initiated the relevant feature.
• Push Notifications: If you opt in, we send push notifications about delivery/appointment scheduling, order status, resupply eligibility, and billing. You may opt out at any time through your device notification settings or within the App.
• Device Identifiers: We collect your device model and type, operating system and version, platform, App version and build for analytics.
• App Usage Data: Screens visited, load events, network connection status, selected actions like order submission, errors, crashes, performance traces are collected to improve App performance and user experience.
• Biometric Authentication: The App may use Face ID or Touch ID to authenticate you. Biometric authentication data is processed on your device and is not transmitted to or stored by us.

Sources of Personal Information

We collect personal information from the following sources:

• Directly from you, when you create an account, use the Services, complete forms, or contact us.
• From your devices and usage of our websites and mobile app, through cookies, pixels, and similar technologies (see Cookie section below).
• From our service providers, including when we engage them to enable certain technology and when they collect or process your personal information on our behalf.
• Certain of our Services offer online shopping functionality powered by Shopify Inc. ("Shopify"). When you make a purchase or interact with a shopping feature using those Services, Shopify may collect information about you directly, including contact, payment, and transaction information, in accordance with Shopify's own privacy policy, available at https://www.shopify.com/legal/privacy. We encourage you to review Shopify's privacy policy to understand how Shopify collects, uses, and shares your information. Our collection and use of information you provide in connection with a purchase is described in this Privacy Notice.
• Our mobile app, MyApp+ is distributed through the Apple App Store and/or Google Play Store. Each platform collects certain data pursuant to its own privacy policies. We are not responsible for Apple's or Google's data collection practices. 3. MyApp+ Permissions

MyApp+ requests the following device permissions. You may grant or revoke permissions at any time through your device settings. Revoking a permission may limit the functionality of certain MyApp+ features.

• Camera: Enables scans of insurance card, registration QR code, and driver's license barcode. If denied, camera-dependent features will be unavailable.
• Notifications: Enables push notifications for delivery/appointment scheduling, order status, resupply eligibility, and billing. If denied, you will not receive push notifications but may access notifications within the App.

4. AI Mask Fitting

We offer an optional mask-fitting tool in MyApp+ that uses a scan of your face to measure your facial features and identify CPAP mask models likely to fit you. The tool is operated by SleepGlad (VGM Group, Inc.), not by us. When you choose the mask-fitting feature in MyApp+, it opens a browser window inside the app that is operated by SleepGlad. We provide SleepGlad with an identifier that lets us connect its recommendation to your account. SleepGlad asks you a set of questions about your therapy needs and uses your device's camera to capture images of your face and create a scan of your facial geometry. That scan is biometric information. SleepGlad collects and processes it on its own platform under its own privacy notice. We do not receive, access, or store any image, video frame, facial-geometry scan, facial measurement, or biometric template, and we cannot retrieve that information from SleepGlad. From SleepGlad we receive the recommended mask model and size, the date of your fitting, and the identifier that lets us connect SleepGlad's recommendation to your account. Because a mask recommendation reflects information about your health, we treat it, and the fact that you requested a fitting, as sensitive health information.

5. How We Use Your Information

We use personal information for the following purposes:

• Providing, tailoring, and improving the Services: Operate, maintain, and improve our platform and features, including to remember your preferences and items you are interested in, to enable you to post reviews, and to create a customized shopping experience for you, such as recommending products related to your purchases.
• Account management: Create and manage your account, authenticate your identity, process transactions, as well as process purchases, returns, exchanges or other transactions.
• Communications: Send transactional notices, product updates, and support responses.
• Marketing and Advertising: Send promotional communications where you have opted in; show online advertisements for products or services on our websites, in our app, or on other websites, including items you previously purchased or added to your cart.
• Analytics: Analyze usage patterns to improve our Services.
• Security and Fraud Prevention: Detect and prevent fraud, abuse, security incidents, and illegal activity.
• Legal compliance: Comply with applicable law, legal process, and regulatory obligations.
• Business transactions: In connection with a merger, acquisition, or sale of assets, personal information may be transferred as part of the transaction.

6. How We Share Your Information

We do not sell your personal information for monetary consideration. We may share your information as follows:

• Service Providers / Processors: We share information with vendors and service providers who perform services on our behalf (cloud hosting, analytics, payment processing, order fulfillment, email delivery). These parties are contractually required to process information only as directed and to maintain appropriate security.
• Business and Marketing Partners: We may share information to support personalized advertising with third-party services based on your online activity with different merchants and websites. Our business and marketing partners will use your information in accordance with their own privacy notices. Depending on where you reside, you may have a right to direct us not to share information about you to different merchants and websites to show you targeted advertisements and marketing based on your online activity. You can exercise your rights to opt-out on the applicable site.
• Legal and Regulatory: We may disclose information to comply with applicable law, respond to legal requests, enforce our agreements, or protect the rights, property, or safety of us or others.
• Business Transactions: In connection with a merger, acquisition, financing, or sale, your information may be disclosed to the counterparty and their advisors, subject to confidentiality obligations.
• With Your Consent: We may share information for any other purpose with your consent.

In addition, MyApp+ may share data with the following app-specific third parties:

• Mobile Analytics Providers: We share information with mobile analytics providers to collect anonymized crash reports and usage statistics.
• Push Notification Services: We share information with push notification service providers to process device tokens to deliver push notifications.
• Chat Service Providers: If you use the in-app chat feature, the chat messages with be shared with our chat service provider.

The Services may provide links to websites or other online platforms operated by third parties. If you follow links to sites not affiliated or controlled by us, you should review their privacy and security policies and other terms and conditions. We do not guarantee and are not responsible for the privacy or security of such sites, including the accuracy, completeness, or reliability of information found on these sites. Information you provide on public or semi-public venues, including information you share on third-party social networking platforms may also be viewable by other users of the Services and/or users of those third-party platforms without limitation as to its use by us or by a third party. Our inclusion of such links does not, by itself, imply any endorsement of the content on such platforms or of their owners or operators, except as disclosed on the Services.

7. Cookies and Tracking Technologies

Cookies are small text files that are placed on your device when you visit a website. They are widely used to make websites work efficiently and to provide information to the website operator. In addition to cookies, we may use similar tracking technologies such as web beacons, pixels, local storage objects, and SDKs (collectively with cookies, "Tracking Technologies").

We use the following categories of cookies on our websites:

• Strictly Necessary: Required for the site and services to function. Cannot be disabled.
• Functional: Enable enhanced features and personalization. May be disabled, but doing so may affect functionality.
• Analytics: Help us understand how visitors use our site. Require consent where applicable law requires it.
• Advertising / Targeting: Used to deliver relevant advertising and track campaign effectiveness. Require explicit consent.

Certain of our Services use Shopify's platform, which may place cookies and similar tracking technologies on your device to enable shopping cart functionality, fraud prevention, and platform analytics. These technologies are governed by Shopify's cookie policy, which is available at https://www.shopify.com/legal/cookies.

You may manage cookie preferences through our cookie consent banner or your browser settings. Note that disabling certain cookies may affect the functionality of our Services. More information on how to modify your browser settings or how to block, manage or filter cookies can be found in your browser's help file or through such sites as: www.allaboutcookies.org.

8. Data Retention

We retain personal information for as long as necessary to fulfill the purposes described in this notice and to comply with our legal obligations. The primary retention criteria we apply are:

• The nature and sensitivity of the data
• The purpose for which it was collected and whether that purpose has been fulfilled
• Our legal, regulatory, and contractual retention obligations (including HIPAA's minimum retention requirement for PHI)
• The potential risk of harm from unauthorized use or disclosure

9. Security

We implement administrative, physical, and technical safeguards designed to protect your personal information against unauthorized access, disclosure, alteration, and destruction. Our security measures include encryption of data at rest and in transit, role-based access controls with least-privilege provisioning, multi-factor authentication, continuous endpoint detection and response monitoring, regular vulnerability assessments and penetration testing, annual HIPAA security risk assessments, and administrative, physical, and technical safeguards for PHI. However, no method of transmission or storage is completely secure, and we cannot guarantee absolute security.

10. Children's Privacy

Our Services are not directed to children under the age of 16, and we do not knowingly collect personal information from children under that age without verifiable parental consent. If you believe we have inadvertently collected information from a child under the applicable age threshold, please contact us immediately at legalservices@adapthealth.com and we will take steps to delete that information.

11. Your Rights and Choices

Depending on where you live, you may have specific rights regarding your personal information under applicable state privacy law. We describe these rights below and explain how to exercise them.

Rights Available Under Applicable Law

• Right to Know / Access: Request a copy of the personal information we have collected about you, including the categories, specific pieces, sources, purposes, and third parties with whom it has been shared.
• Right to Deletion: Request that we delete personal information we have collected about you, subject to certain exceptions (legal obligation, security, fraud prevention, etc.).
• Right to Correction: Request that we correct inaccurate personal information we maintain about you.
• Right to Portability: Request a copy of your personal information in a portable, machine-readable format where technically feasible.
• Right to Opt Out of Sale or Sharing: Direct us not to sell your personal information or share it with third parties for cross-context behavioral advertising. See 'Do Not Sell or Share' section below.
• Right to Limit Use of Sensitive Data: Direct us to limit our use of your sensitive personal information to purposes necessary to provide the services you requested.
• Right to Non-Discrimination: We will not discriminate against you for exercising any of your privacy rights.

How to Submit a Rights Request

We will verify your identity before processing your request. We will respond to verifiable requests within the timeframes required by applicable law. We will not charge a fee for reasonable requests.

Authorized agents may submit requests on behalf of California residents pursuant to CCPA/CPRA. We may require written authorization or power of attorney and may verify the agent's identity independently.

Do Not Sell or Share My Personal Information

For California residents and residents of other states with enacted opt-out rights, you can exercise your right to opt out of the selling or sharing of your personal information on the applicable site.

11. Changes to This Privacy Notice

We may update this Privacy Notice from time to time to reflect changes in our practices, technology, legal requirements, or other factors. We will post the updated notice on this page with a new effective date. For material changes, we will provide more prominent notice, such as an in-app notification or email, where required by applicable law. Your continued use of our Services after the effective date constitutes your acknowledgment of the updated notice.
